Description

• Conduct outreach to identify high need women and infants • Plan and utilize creative outreach strategy events • Plan and facilitate educational presentations • Conduct initial intake and assessment of participants • Assist participants in completing CAMBA intake applications and consent forms • Manage a small caseload • Prepare service plans and goals • Assist participants in attaining goals by identifying and locating community resources • Monitor participants' progress and document via progress notes • Act as liaison and advocate for participants • Follow-up with participants after successful completion of goals • Escort participants to appointments and act as translator if necessary • Assist participants in completing applications for benefits and entitlements • Provide required data and narrative information for reports

Requirements

• High School diploma or GED and 4 years of applicable experience • Some evenings and/or weekends may be required
