• The Human Resources Generalist oversees daily functions of the People department including recruiting and onboarding, supporting compensation and benefits, and enforcing company policies.

Human Resources Generalist

Develop and implement a robust quality program for WelbeHealth’s PACE services.

• Partner with all WelbeHealth market QI Directors to monitor and standardize the QI program for WelbeHealth PACE programs across the enterprise
• Collaborate with the Medical Directors and QI team members on the development and implementation of QI program initiatives, and leverage best practices among WelbeHealth PACE programs
• Collaborate with our CMO and Medical Directors to develop and standardize enterprise metrics in clinical excellence, quality, and patient safety in alignment with all policies and procedures, as well as local, state, federal, and CDC guidelines
• Collaborate with WelbeHealth’s Compliance team to monitor and improve compliance issues, corrective action plans, and the Fraud, Waste, and Abuse program
• Collaborate with Executive Director and Medical Director during PACE Program's Federal and State audits from facility review through corrective action plan development and monitoring
• Oversee the grievance and appeal process, including the delegation of investigating all quality reviews for potential quality of care concerns, as well as quality of service and access issues, including collection and evaluation of appropriate data
• Support PACE program procedures to process grievances and complaints in accordance with applicable state and federal regulations, compliance standards, and policies and procedures
• Develop and maintain relationships with state and federal regulatory agencies, including the NY State Department of Health and Centers for Medicare and Medicaid Services

Director, Quality Improvement 

Provide comprehensive medical care for elderly participants in a community-based setting.

• Assume accountability and responsibility for the medical care of WelbeHealth participants, overseeing the medical service team to promote quality and outcome goals.
• Monitor and lead WelbeHealth clinic operations to assure integration of medical services with other participant care services for participants.
• Recruit, train and supervise PACE Physicians, Nurse Practitioners, Community-Based PACE Physicians and other contracted medical staff.
• Perform comprehensive assessments for participants and coordinate with the IDT to develop a comprehensive care plan for each participant
• Be an engaged member of the IDT, communicating participant changes, collaborating on care planning arrangements, and coordinating 24-hour care delivery
• Work collaboratively with network providers and other members of the IDT to ensure effective care coordination, particularly around hospital discharge, short-term, and long-term placement
• Oversee participants’ use of medical specialists and inpatient care
• Participate in rotating night and weekend call
• Review outcome and utilization data, including adverse events, hospitalizations, and use of specialists, in collaboration with the IDT to identify opportunities for improved care and operations
