• Manage call flow and services for NY Connects • Support a team to meet community needs • Develop and implement outreach plans • Attend community events to promote services • Create and manage care transitions • Oversight of staff ensuring adequate services • Data entry into PeerPlace for service tracking • Monitor program's call system • Analyze and track program outcomes
• Bachelor’s degree in Social Work or related field • 1 to 3 years’ experience in health and human services • Proficiency in Google Suites and Microsoft Office • Excellent time management, attention to detail, and organizational skills • Strong communication skills
